Terms and Conditions: Acceptance of the course depends on the course availability and on a first-come-first-serve basis. Participants should be medically fit. Those with medical conditions such as asthma or slipped disc and are advised to consult their own physician prior registration for the course. Pregnant women are advised not to participate at least 6 weeks after child birth.

Policy: Once registered, no cancellations are allowed; Deferment to another date must be notified within 15 working days before commencement of the course. However, substitute participant is accepted.

*I am / We declare that I will not hold the management, chef instructors, instructor or any other personnel of Medaid Healthcare for any personal loss, damage/injury, cost or any other liablility whatsoever, as a result of my participation in this training.

*I am / We declare that I am not pregnant, nor do I have any medical conditions such as injuries in the spine, knee, wrist inclusive of other injuries that could be aggravated by participating in this class.

*I am / We are warrant that all information provided are true and correct. *I am / We understand that the Medaid Healthcare Pte Ltd has the right to reject *my / our application without any explanation.

*I am / We are fully aware that safety is everyone’s responsibilities. *I am / We hereby declare that *I am / we shall not hold Medaid Healthcare Pte Ltd responsible for any mishap during the course.
